1. Edit your survey with the page editor.
   
2. We’re going to add an existing page header to the page to be sure it’s got the expected layout - if you’ve already got one then skip to step 6. Otherwise open the question toolbar at the position you want to add the page header.
   
3. Open the template selector popup.
   
4. Select the page headers tab.
   
5. Select a page header to add it to your survey.
   
6. Open the image editor popup.
   
7. If it's showing the foregorund image (the logo), we need to switch to the background image.
   
8. Open the image selector popup.
   
9. Select the desired background image by double clicking the appropriate image. Note: you could upload an image of your own.
   
10. Update the survey with the amended page header.
    
11. Double click the logo to change it.
    
12. Open the image selector popup.
    
13. Select the desired foreground image (logo) by double clicking the appropriate image. Note: you could upload an image of your own.
    
14. Update the survey with the new logo.
    
15. Save your survey.
