To add password access to your survey you need to set it to require a password (steps 1 – 5) and then add the password question to your survey, so the password can be typed in (steps 6 – 9).

Open your survey with the page editor:

Go to the Survey options tab.

Display the advanced options.

Enter the password required to respond to your survey.

Update your survey.

Now we need to add the Owner Password question to the survey so the password can be typed in. Open the toolbar at the position where we want to add the question.

From the question’s toolbar open the Template Selector popup.

On the Template Selector popup switch to the "Public templates" tab.

Scroll down the question templates and locate the “Owner Password” question. Click to add it to your survey.

That is all you need to do - you could leave it there and save your work - but we are going to style it a little, so double click the question to edit it.

Open the Options popup.

Change the layout from vertical to horizontal.

Close the Options popup.

Now we're going to remove the word "access" (because the text is too long for the page) and then regenerate the question.

Save your work.
That’s it; now if you activate your survey and try to respond to the active version, the correct password will be required.
