Go to the account section of the site: either select “Account details” from the “Go” drop down menu or click on your email address.

Open the Survey Friendly ID prefix popup.

Enter your desired prefix

Once your prefix is set, you need to ID your survey: return to the survey manager page.

Edit your survey using the page editor (if your survey is active select “Open it anyway” from the popup and skip to step 7).

Select the “Survey options” tab.

Click the “Show advanced” button and scroll the page down. Enter the ID for your survey; if you want this to be the default survey that is launched from your internet address then enter a dash (minus) sign.

Click the "Update" button (returns to the page editor) and then save your survey.
When you activate your survey you will be able to access it by the friendly survey ID, e.g.:
http://takesense.com/yourCompanyName/SurveyID
or if you set your survey to be the default, i.e. you entered a dash (minus sign), then:
http://takesense.com/yourCompanyName
Note: it does not matter whether you type in upper case or lower case letters.
